Incident Report

If a mishap occurs while on Parish grounds, it needs to be reported to Parish staff as soon as possible. Examples of reportable incidents are when a person falls, passes out, gets sick, etc. Basically, ALL incidents, with or without injury MUST be reported. We have Incident Report forms available in prominent locations around our properties but you can also use the online form below.
